Dear queriest, I think that Police verification is not necessary.You will find the proforma in rules of Bar Council of India. for rules search google.

There is neither any template nor requirement for police verification when you want to get enrolled as an advocate.
Where from did you find that an advocate need to be verified by police before enrollment??.
