Marriage certificate and name change after marriage

Dear Sir,
My name is Manik Prabhu from Saketh, Hyderabad.
Am married in March 2001. Please find below the following points:
1. Earlier to marriage, my wife's name was "PREMA' (includes in all certificates)
2. After marriage it was changed to
D.Manjulatha. Even in invitation also name is printed as Manjulatha
3. I have taken Aadhar card, PAN Card etc on Manjulatha only.
4. Now I want to obtain a passport on D. Manjulatha only.
5. How to get marriage certificate and does it helpful for getting passport.
kindly help with your valuable advices.

In marriage application, you can mention the name of your wife as D.Manjulatha alias Prema with her parent surname. and both you and your wife have to execute affidavit stating that the name of the wife is Prema before the marriage and after marriage is D.Manjulatha and both names belong to one person and get it be notarized by a Notary.

As there is major change in name / first name change passport officials may ask for respective Annexure and Advertisements in two news papers.

Obtain marriage certificate.
Submit prescribed name change Affidavit.

It is better to obtain the Marriage Certificate by applying with the SRO/RO of the place where marriage taken place for change of name of wife after marriage in a prescribed form along with a notarised affidavit, marriage invitation card and photo of your marriage. Registrar will issue the modified certificate after enquiry.
It is advised to apply the passport after obtaining the modified name change certificate.
