Passport-permanent address related

Sir,
My present PERMANENT ADDRESS is about to change ,
but how can i change this permanent address from my PASSPORT records to my new permanent address??

Please reply sir, its urgent

instructions taken from net are as under:

It is up to you to change the address. It is always better if your passport indicates your current address. For this purpose, you need to fill up form-II and submit it along with valid proof of your residential address at Passport Office of the area of where you reside.
