Que.1.Dear Experts Please provide me with the proforma of Application to be made in the Family Court to Transfer a New case(RCR) from Junior civil Court(in the Suburb) to Family Court(Where Divorce Case is already in process) in the Dist HQ?

There is no specific performa for this purpose but it should be moved a simple application. mentioning the case pending before the family court (as mentioned in quarry) with date of hearing and should mention that it shall be in the interest of justice, if both cases are heard by the similar court.
