will my Passport be rejected or will i be fined?? please hel

Respected sir/madam, My name is VADARI V MARUTHI PRASAD in all my documents. But in my birth certificate it was written VADARI MARUTHI PRASAD without V When I applied for passport in PSK they asked me to write a declaration in this regard that the BC is mine only and I am only child of my parents . after police verification happened I found out that they sent adverse report mentioning name difference.in BC now I am getting a show cause notice for it again. How will I be treated - by a rejection or with a fine or will some affidavit or declaration should do the job and get me my passport.
