Format of certificate under bankers books evidence

You can ask your banker to issue a format or search for the desired format in the internet or google search, formats are not supplied here

Sir, I tried searching on the inter, didn't get. Also the banker is not co-operating, hence though of taking help of the esteem group here.

This is a local issue hence you may have to look for some assistance through local sources. 

You cannot file a suit against the bank to direct them to furnish the desired certificate's format.
