Need help to apply passport who is separated but not legally

HI,

I am planning to apply for new passport for my mother who is separated but not legally. 

We don't have any marriage certification to submit to the passport office.

I called to Passport helpdesk and i have explained the same and they suggested to use Annexure-K in place of marriage certificate,  but ANNEXURE-K only talking about re-issue of passport.  

Please let me know your suggestions to proceed.
