Format of paper publication - regarding section 9

Dear Expert

Could some one please suggest what should be the content of the paper publication for the notice served by court regarding section 9 to my wife. it would be great if some one can send me a format else guide me if i can customize the content with basic details instead the complete order from court. please advice.
