Hi,
My Wife is appying for new passport. She wants to use her maiden name (Sumana Das).
In passport office they want Annexture D, as in passport application my name is written in Spouse section. As per my understanding Annexture D is used only when there is a change in name after marriage and an women wants to use the changed name in passport.
I don't understand why they want annexture D in my wife's case.
Can any one please suggest what all documents are required if someone wants to keep her maiden name in passport even after marriage?
