Dear Experts,
My son was born on 20.11.1989 in Chennai.
The certificate issued bears the name Amod whereas afterwards we have named him as MOHIT JAIN. As per school records.
Also the father's name entered is wrong.
Now in order to apply for passport what are the formalities required to rectify the Birth certificate.
As I now stay in Bangalore rectifying the certificate would also be a problem.
Is there any form of affidavit, which can serve the purpose.

Please decide as to what name your son must be assigned now. If you want to retain the same as per school certificate, you can get the name published in gazette and it would legally hold goog for Passport purpose. Submit all relevent documents for the same to the office located at Mount Road (Next to Higginbothams Books stall located at the premises of Printing and Stationery Department). The process will take at least three months as they insist for paper publication.
