Proof Affidavit

In a consumer court complaint, after the opposite party has filed their version, the judge has now asked for both of us to file a Proof Affidavit. Please guide me in what all could be included in the affidavit apart from the basic facts of the case like:
My name, occupation, address and that I am the complainant
The complaint against the opposite party
Evidence submitted by me during the complaint
Relief seeked by me.

Is there anything else that is to be added to the Proof Affidavit?

It could be proof of transaction or repudiation of the same by the other side.

Proof of affidavit means it is an examinatin of chief by way of an affidavit.
You have to say thru., an affidavit whatever you have stated in your compalint.

In complaint, you have stated many facts and your claim.

Proof affidavit is series of facts and relevant documents to be produced into Court as Exhibits in appropriate places.
