Urgent mother's surname issues for baby's birth certi

Dear Experts,

 

I am in urgent need of advice.

I am having a minor baby of 2.5 months. I want a passport issued asap so that i join my husband overseas.

Issue is

 My passport has my surname pre marriage. and my baby's birth certificate has post marriage surname.

    most of my docs have my old surname except my baby's birth certificate and I want to use both my names as when necessary.

1. Do I need to change my surname on my passport for my baby's passport? (though I am not willing to change my surname on my passport as many bank accounts are related on this document)

or

2. Do i need to correct my baby's birth cert with my previous surname? If so how , will it be a simple process since I am correcting only my surname.

or

3. Or while going for passport a sworn affidavit stating same person will be sufficient.

generally the surname of Hindu female will change after marriage but the certificates etc the parental surname remains.  so execute an affidavit that your name is xxxxxxxx before marriage and after marriage the name is changed as xxxxxxxxxxxxxx on stamp worth Rs.10/- and get it attested by a Notary.  it will sufficient.  this is for information only.
