removal of pharmacist name from drug license

Hi i worked with shalby hospital indore as a pharmacist and gave my pharmacist registration to make drug sale license in 2015. I left job in 2016 and from that time to now i am requesting to remove my name from the licence but no action is taking by hospital and drug inspector as well. Now i am mentally harassed. Please suggest how and where to complaint for both hospital and drug inspector.
