salary issue.

Respected sir, I am Dr.shyamprakash working as a government doctor in the salem district, Tamilnadu. recently counselling I opted for a new place, I received the transfer and posting order on December 25. I was relieved from the old station on January 31 forenoon and I joined in the new station January 31 afternoons, almost 40 days over till now in my old station hospital superintendent not ready to issue my last pay certificate, even I request orally, as I am physically challenged orthopaedically handicap it's my only source of income. I am dependant on my salary, I am unable to lead my life and I am facing difficulty, I think they are expecting bribe to issue last pay certificate, how to deal this? please guide me.
