Obtain your proper certificate from mosque for Conversion to Islam with seal and signature on letter head of Masjid.

Thereafter you can sign an affidavit stating that you have converted to Islam on her own free will and also adopted new muslim name………….

You may file for change of religion, change of name in government gazette.
