Adoption petition

Hello... Dear Friends.. i want to file child adoption petition before the civil court if any one filed before that plz send me the copy of petition memo so i can draft it.. short facts of the case is :- lady is widow having kid the age of 5 yr and remarries with the person, child original biological father is expired and now new father want to legally adopt the child.....
