Format of cm for early hearing

Dear All Respected Sir/Madam,

I am a petitioner in person in a case and want to file a c.m for early hearing. I am not able to find any c.m. online. It shall be great help if someone can provide me a copy of same in pdf or word format or anyone filed any c.m. can provide me a scan copy of same.

Help shall be higly appreciated. Pls friends help me
