Changing mother's name in birth certificate

Hi,

I stay in Pune. The birth certificate of my child has incorrect mother's name. By mistake, the hospital authorities have put my mother's name instead of child's mother (i.e. my wife). What is the procedure of correcting the mother's name.

I have requisite letter from hospital authority.

2 letters from coporators

Affidavit

Original Birth certificate.

Please guide me and help in follow proper procedure.

You should mention correct name where ever required. also ask the hospital authorities to issue a duplicate/corrected certificate.

Just a small clarification, the birth certificate was issued by municipal office. The hospital had send a form bearing the details. However, they had put incorrect mother's name.

Under Birth Registration Act such mistake could be corrected by submittimg the requisite documents you have

 Apply to municipal corpn with all documents.

hello... first u need to take correct details from the hospital and approach the muncipal office to get it corrected.
