judgement in matter of insurance co.reject claim

hello..
any judgment against insurance co.( whereas consumer have to insurance and they have admittetd in hospital afterthat discharge the insurance co. doesnt pass claim)

There are umteen judgements but on what ground Insurance Company failed to settle the claim?
