Copyright under pseudonym

Hi, 

I am writing a work of fiction (currently unpublished) under a pseudonym. I do not intend to reveal my real identity. I need to self register for copyright under the pseudonym or pen-name. I am willing to reveal my real name to the copyright office but not to the public at large.

In the copyright registration form there are three distinct names to be provided - Applicant, Owner and Author. What name should be put under each colum (real or pen name)

Applicant -

Author -

Owner -

so that I am on correct legal footing and also protect my anonymity? In which column to put the pen name so that I get the copyright in the name of the pen-name?

Also, can the "address" field be skipped in the column where I fill my pen-name because that would defeat the very purpose of having a pen-name?

Please guide.

You should make a company and every copyright to be needed with Pseudonym can be registered with that company's name. You can make the pseudonym as same as the company's name.
