Father unable to sign due to illness

My father is 59 years old and suffereing from alzheimer for last 6-7 years.
This is a brain functionality disease in which a person's memory keeps on degrading and finally completely lost.
Person faces problem in identifying, calculating and decision taking.
Beacuse of that they are unable to make their own signature.
Now I want to know how he can with draw or transfer his own money.
He also need some physical shares transferred to Demat or encash.
We also need to transfer the house to my mother's name.
Pl suggest how to resolve this. Is there any legal document to make signature as primary identifation and transferring assets to my mother's name.

The Tehisldar may appoint commissioner to visit at home.

The POA in such case may be allowed and witnesses should sign,

Pl describe how to approach Tehsildar and which form we need to fill.
