Spelling mistake-Birth Certificate

My name is MADAN. It's same in all documents except birth certificate, in which its mistakenly typed as MATHAN. I would like to apply for passport (born after 1989) . What's the procedure. Help pls

U have to apply in prescribed form to gazate office in ur state, ur changed date of birth or name will be published in the gazate. U can apply for more than one copy of that gazate.
