Clarification for change of name

Hi,

I am Vijayakumar, my wife was known as "Subashri Alias Divya. R" before marriage. And now after marriage, we have changed her name to" Subashri.V" in Tamilhadu State Government Gazette. We are going to give advertisement in 2 newspapers. Is it enough to change the name in passport and to get VISA for abroad.

You are required to furnish joint affidavit of the couple in prescribed format with couple's photo affixed for passport and visa purposes.

Thanks for your reply. Can you please tell me in detail how to get that affidavit. Already I have marriage certificate also.

You approach any local lawyer for preparation of affidavit and same can be submitted at passport office and acknowledge the same.There ends the matter.For format of affidavit search in passport website.
