Go to a notary and get your name changed. Notary will give you an affidavit and then you make copies of that affidavit and give an notice in the local paper where you live declaring the change in name. Preserve the original copies of affidavit and newspaper copy in which such notice appears.Then use this affidavit copy with other documents that you submit in the passport office.

Sir, tell me one thing....You said "No big issue" but will see any kind of issue in future ? If no then it is ok for me.

Passport document query for married ladies:

 

 

Hello Passport/VISA lawyers and experts,

My mother wants to apply for a fresh passport and she is 53yrs old. She intends to get it with her new surname (as she is married).
Her birth and 10th grade certificate shows her old surname.

She currently does have the following docs. in support of her new name:
1) Adhaar card
2) PAN card
3) Voter ID
4) Running bank account (SBI)

She does not have the following docs. in support of her new name:
1) Marriage certificate


Can someone amongst you experts please advice me If she needs to get a new marriage certificate issued OR Annexure-D will suffice the purpose?
