Multi-person affidavit

can more than one person jointly make an affidavit ? If yes, what will be the form of such an affidavit ?

The Affidavit is to be deposed and sworn in as individually,but Husband and wife can  mutually submit in Mutual Divorce cases or any title/name change cases.

Suppose I write an affidavit on behalf of myself and two others A and B. Who shall sign the affidavit - myself alone or all three of us ?
