Father's name in the birth certificate for unwed mother

Hello,

I wanted to know what is the law concerning the father's name in the birth certificate of a child born out of wedlock. My partner is a non-Indian citizen. We are not married to each other or anyone else. We are single. We would like to include my partner's name in the birth certificate of our child as the father. Our child is due towards the end of this year. We are planning to have the delivery in Calcutta or Bangalore. My questions are:

 - Can we just provide his name to the hospital authorities and his name will be included in the birth certificate?

 - Or do we need to do any paper work, affidavit, etc. beforehand so that his name is included in the birth certificate?

 - Does the procedure change from one state to another?

 - Is there anything particular that needs to be done?

  1.  No requirement that father has to be husband for purpose of birth certificate. You can enter his name without any problem.
