Pls advise.....
My Surname-BASAVA
Given name-Divakar Srinivas.
DOB-10/05/1980
Am residing in Chennai from 08/08/05 and got my passport renewed in Chennai only.
Now here is the problem,in my SSC(10th) Certificates my name is printed as D Srinivas Basava(passed-1995) and also in my 10+2(passed in 1997).As per this my professional course certificate also had name as D Srinivas Basava.
And rest of the documents like passport,driving licence,pan card and all my name as Divakar Srinivas Basava.
This is the way i want in rest of the documents.
Coz i want to change my name in my proffesional certificate for that they asking either name change or gazzette certificate.Pls advise.If has to apply for name change where shall i apply..i mean In Andhrapradesh or Tamilnadu.
