Bar enrollment in maharashtra & goa

I graduated law in 2017. Presently, I am working with a company as a Legal Executive. Originally I hail from Patna, Bihar but presently I have shifted to Mumbai for my job. For Bar enrollment, one is supoose to submit police verification form, since I am staying here with my uncle. I have few queries regarding the same: 1. Do I need to execute any agreement or affidavit with my uncle that i am staying here with him and his family? 2.
