Minor changes can be made in the names mentioned in the birth certificate. Total changes creating a new name are not legally permissible in order to avoid fraudulent applicants.

You can make a minor change of name of father in the birth certificate in India by moving an application for the same before the authorities where the birth of your child has been registered.

You are advised to approach the authority where the birth of your child has been registered and move an application for the correction of the same, stating therein that your name is spelt as ……….. In the birth certificate whereas the same has been shown as such in your …………….. certificates, and in his personal identity proofs.

And that you want the said correction to be made want the said correction to be made in the birth certificate.

Attach attested photostat copies of your credentials where your name has been mentioned and spelt as per the change.

Draft and get attested an affidavit on the above lines and attach it with your application for correction of the same.

When corrected, you are advised to obtain four or more copies of the corrected birth certificate as you may deem fit as the same will be of great use in future.

The above is my personal view in respect of your question and not a legal advise to create any client - attorney relationship amongst us.You can take your own independent decision irrespective of my above answer as you are better to judge and opt for the future course in the circumstances of your case.

Submit an application on prescribed format with sworn affidavit for doing the needful to the issuing authority.

Registrar Birth & Death is the competent authority wherein such application supported with a duly sworn affidavit is required and if no relief is granted then the appeal lies with SDM.

If the omission is due to the reasons of the authorities concerned and you had clearly made out the correct names in the application with proper and prefect spelling, then it is the fault of the authorities concerned.
You can submit an application seeking correction of name of the father based on the documentary evidences to prove that your previous application had correct name and spelling whereas due to an error in the system or office, this error has crept in hence request the authorities to rectify the error and furnish a fresh birth certificate with proper name.
The authorities will certainly rectify after verifying all the records held by them.
However if it was your fault then you may not get any relief from them directly, you may have to follow the name change procedures as per prevailing law in this regard.
