Correction in ssc and intermediate certificates

Respected sir, My name is spelled Chinthapalli Shashindar in ssc and intermediate..now I want to correct my name as Chinthalapally Shashinder Reddy....there is no "la" in Chinthalapally and there is 'a' instead of e in shashinder...no Reddy...now iam persuing bsc 3 rd...how to change it..thank you

Such inconsistencies are quite common while writing Indian proper nouns in English and could be explained through an affidavit, as and when required.
