Will my passport be cancelled or i ll be fined? course of action ??

Respected sir/madam, My name is VADARI V MARUTHI PRASAD in all my documents. But in my birth certificate it was written VADARI MARUTHI PRASAD without V When I applied for passport in PSK they asked me to write a declaration in this regard that the BC is mine only and I am only child of my parents . I did it and they allowed the same. Now after police verification happened I found out that POLICE sent adverse report mentioning name difference in BC now I am getting a show cause notice for it again. How will I be treated - by a rejection or with a fine and will some affidavit or declaration do the job to get me my passport or is name change only option ????

Will a plain paper written declaration by me and my parent do or should I produce some other document or affidavit ?? Will my passport be issued after that or will I be penalised
