How to include surname in my passport

Hi
I dont have surname in my 10th certificate and passport.
I want to include my surname "Singh" in passport.
I got married in 2007 and my husband's surname is not "Singh"
How do I get to add surname "Singh" in passport. Which affidavit should I use. Affidavit for Change of name after marriage or affidavit for Change of Name

To include the surname,do we need to publish the same in newspapers?

You should apply with an affidavit for change of name. A paper notification is required for the same.

You should apply to the regional passport officer with an application along with an affidavit for change of name.

FIRST APPLY FOR CHANGE OF NAME - IN 2 NEWS PAPERS(ONE IN LOCAL AREA AND ANOTHER IN PERMINENT ADDRESSS AREA) PUBLICATION,THESE ORIGINAL PAPERS, ALONG WITH AFFIDAVITE ON NON-JUD. STAMP PAPER(Rs.10/-) I.E CALLED ANNEXURE-E (SWORN IN)ATTESTED BY NOTARY ALONG WITH ORIGINAL PASSPORT.SUBMIT TO RPO CONCERN.

Application should be supported with an affidavit as well as notification/notice in at least 2 daily news papers and the same shall duly been moved to office of Regional Passport Athority of the area from which the earlier passport had been issued. A crossed cheque or DD of Rs. 500/- is also required to be annexed.
