submition the Notary Application form

Dear Sir,

I have asked a query on how to become a Notary License holder lawyer.
I have got the reply and the downloaded the application form from this website,

but I need to know where this form needs to be submit ? And Pl also clarify wether for Notary registration can I apply to UP state govt. or central govt or both,as I heard that UP state Govt. has stopped to permit Notary lawyer applications.

A person may make an application for appointment as a notary addressed to such officer or authority of the appropriate government as that government may, by notification in the Official Gazette, designate in this behalf.The competent authority generally is the Law Department at the state level and the Ministry of law at central level.
