I lost my School leaving certificate, so what is the procedure to apply for a domicile certificate.

domicile certificate is needed to prove that you have been staying in a state for 15 years or so . if you were born in that state you dont need a domicle certifcate .
The prescribed application forms are either available online or from the local authorities, i.e. the Sub-Divisional Magistrate/Tehsildar's office/Revenue Department/District Collector's Office, or any other authority as specified in the State/UT of your residence. You will be required to produce proof of continuous residence in the State/UT for a specified minimum period, or that of holding land in the State/UT, depending on the rules in the State/UT concerned. Documents for proving your identity, attestation of the form by officers with the necessary authority, school certificates and the Tehsil Enquiry Report may also be required

if you want a domicilliary certificate only,you can follow the advise of Mr.Ajay.
you can also approach the board of examinations of your state for a duplicate school certificate.

Approach your institution for a duplicate Leaving Certificate. In some states, there is a practice of filing a police complaint for loss of certificate and to produce the copy of FIR along with an affidavit to get a new certificate.
With regards to the domicile certificate, i agree with the advise from Mr. Sethi.
