Employee is not serving notice period

hello,
can any one give me sample draft of the notice that employee is leaving company without serving notice period or paying anything in lieu of notice period. i just need legal notice format for the same. please help asap. thanks.

facts of each case are diffrent . so no typical format can be given . it has to mention reasons why you are leaving company on account of illness etc

It is very difficult for anyone to draft legal notice without having any detail. So, unable to provide the same.

Better hire services of some legal expert at your place to prepare suitable draft with particular reference to conditions of service of employees set by your organisation.
