about notary afidavit

I want take a cooking gas connection. For this dealers are advised to me that bring a notary afidavit in ur name. then u can be provided the connection.Then how can i do this afidavit plz advise .

pls buy a 20 rupees stamp paper. that is all is needed for affidavit. in that write your name, address, age, and write the purpose of the affidavit (gas connection).

after getting the matter typed, go to the notary and get the signature.

best is to take the stamp paper to a notary, he/she will type it in the required format and put the signature, as you are unaware of how and what should be written.
