which documents needs to be attached in chief affidavit

I have applied for divorce on the grounds cruelty. My husband filed counter petition for my divorce petition. Can we file chief affidavit. Which documents are marked in chief affidavit. What are the specific things chief affidavit contains. Do I need to provide any witnesses in affidavit. I didn't filed any dvc and 498a against my husband as I don't want to get into problems raised because of my husband. Please tell me in detail about chief affidavit. And the list of the documents which will be marked during that time.
