No need of specific format...just write

Courts's Name

Case No.

Parties name

Application for Adding Name of the Parties under Order 1 Civil Procedure Code.

Just Simple Request For addition of name of parties along with there Proper Postal Address.

Prayer.

Signature.

You can file your application in this way.
