I have applied for Passport with given name as "PRASHANT KUMAR" and surname as "TRIPATHI".
My father uses this surname. But in all documents my name is mentioned as "PRASHANT KUMAR".
How can I add my surname considering "KUMAR" as middle name.

When applying for passport change your name with government gazette and thereafter give public notice.

The passport officials consider your name as per X class certificate and your birth certificate. If you want to add surname, you have to obtain gazette notification certificate. For that you have to apply in gazette department at your state capital city.
