At what stage should we give documentary evidences?

As there are different stages in a civil suit

P: Documents

Q: ISSUES

R: Evidence

at what stage we should give evidence, ie., documents like general old wrtings, old 7/12 extract(not new), old revenue records(Not Public documents any more as records are not available), old cases between ancestors(not res judicata)

what is the difference between documents stage and evidence stages? is document stage is about submitting the main document which is the matter of issue like a bond between parties of which is the issue? or is it all evidences which are documents?  .ie all evidences which are not oral like are of paper in nature?

Further do we have to submit any application or a list of evidences that we are going to rely upon? so that it gets exhibited. Like I have seen some documents submitted have got exhibit numbers and what to do if the documents have not got exhibit number and which are important evidences for your case.
