correction of SSC memo

sir actually my name registered in school is NUTANAKANTI PAVANKUMAR but I got my SSC memo with NUTANKANTI PAVAN KUMAR But my Real name is NUTHANAGANTI PAVANKUMAR now I completed my post-graduation with NUTANKANTI PAVANKUMAR SO how to correct my name pls advise me sir

The name is as per forms filled and submitted by you and documents attached e.g; birth certificate..

For any change approach office of Principle of school and they shall guide and provide requisite forms for procedure applicable as per Board..

Likewise  for Univ..
