Rti for 498a wife

My 498a wife given affidavit to passport office-chandigarh , that she is residing at her parents home even after marriage .

I need copy of that affidavit, Please advise me to whom i need to file RTI and what i need to write in RTI application?

Note:- I have a scanned copy of her passport with me.

You need not to file RTI, but u can apply for certified copy of that affidavit.

You make a simple application to THE PUBLIC INFORMATION OFFICER, OFFICE OF THE REGIONAL PASSPORT OFFICE, CHANDIGARH"

giving details as to your name, address, phone no. informaion required, period to which it belongs to quoting her passport number etc, with Rs.10/-Indian Postal order for RTI fees. and Rs.25/-IPO for postage. send this letter by Regd A/d. With in 30 days they are supposed to give you the information.
