affidavit for service of notice

hello, I am not a advocate but I perusing a case on my own in maharashtra state consumer forum. I need to submit affidavit for service notice to opposite party. can some one share the format and explain the requirements around it for e.g is it on stamp paper, notarized etc.? pls explain

Filing a copy of notice issued to opposite party with such service proof like postal acknowledgment card, postal receipt or delivery status downloaded from indiapost website are enough service proofs. The purpose of seeking such service proof is that enough time was given to OP, pointing to deficiency and seeking remedy, which was ignored or responded by OP. If OP has responded in writing, that copy of letter from OP is itself a service proof.
