i have filed a case with the consumer court. since the other party has failed to appear on multiple occasions the judge said that he would pass an ex parte order. he asked me to file an affidavit. i would like to know whether this affidavit is called EX PARTE AFFIDAVIT and whether all pages of the affidavit need to be notarized.

Why are you worried what it is called. Just write your story and get it signed by an advocate and file it. As other party does not appear in court, you will get favourable orders.
