Urgent need advice for alias name

Respected Sir,

I need advice & valuable suggestion from your side . In 2013 , In my 10th and 12 marksheet my name were changed from "Ashish Kumar" to "Ashish Kumar Alias Ajay kumar" (Marksheet attached). Now i am very confused which name should i used further in all my Govt. Certificates, educational qualifications and passport so that i did not get any problem because i am facing problem by using two names, some online form filling portal did n't accept two names.

Please give your valuable suggestion i shall be really thankful to you.

As per my knowledge, original name has to be used in Government records. Alias has no relevance.

In my birth certificate my name is register as Ajay Kumar and in my schooling my name was Ashish Kumar. In 2013, to make the same identity CBSE changed my name from Ashish kumar to Ashish kumar Alias Ajay Kumar. So should i go with Birth certificate name i.e Ajay Kumar in all my govt documents?

Birth Certificate is the most authentic proof.

Please tell me the legal procedure to change my name from Ashish kumar alias Ajay kumar to Ajay kumar in all my govt docs like Aadhar card, PAN Card etc.
