Rental agreement closure form

I let-out couple of my houses and rental agreement made for both the tenants. Now one of them wants to vacate from next month. Are there any rental agreement closure forms available?

 I need to cover couple of things in the form of closure of agreement,

 1.                  Rent recovered from the tenancy period, which will be additional income for IT calculation. 

Since I have not provided rental receipt to the tenant and he paid rent on cash, do I need to make only self declaration of the rent received or by the way of closure agreement, I can indicate how much money I have received by the way of rent for that FY.

 2.                  The tenant paid advance in cash and insisting me to pay cash while he vacating the house. Will it possible to cover the same in the rental closure form.

 Please share any standard rental closure forms available.
