Rejoinder affidavit for rcr counter

I filed RCR in jan 2012 and in the month of nov 2012 opposite party filed counter with all false allegations,at that time my counsel said to the judge in the next date we were going to file chief affidavit,after  seen the chief affidavit,i came to know that it was the replica of O.P,I asked the counsel that,why we are not deniying false allegations in the counter,i dont know what his intention is,but i want to file rejoinder for the counter,

So my questions are:

1)under which order/rule we can file rejoinder

2)is it necessary to file I.A for permission to file rejoinder

3)if judge not given permission to file rejoinder,then what should do,because prior to 90days they filed counter.

4)what is the difference between chief affidavit and rejoinder affidavit.

5)need sample formats for permission to file rejoinder,and as well as effective rejoinder format.
