Possession letter through self declaration for flat -format

Hi ,

Please help me with the format for 'Possession Letter through self declaration'  for my flat .  Till now i used to show my Bank Provisional certificate to claim my housing loan interest under Self Occuiped Property. 

In my new company along with Provisional certificate they also require either 'Possession Letter from Builder or through self declaration from the employee',  as per IT circular no 8/2012, dated 5th October 2012.

Kindly help me with the sample format for Self Decleration of Possession Letter for my flat , I misplaced the Possession Letter given by the builder.
