yes
he has to submit the sample for getting certificate frm the trademark and patent right department of the govenment. u have to fill all the details which by prescribed form by the department along with the samples. they test the quality by their own lab .

which are the concerned offices to submit the application, samples, prescribed forms etc?
do such offices situated in every state?

yes every staate have the branch offices
just go and ask them abt that then they will guide
