Name in passport

Hi there,

We have named our girl baby as M.S.Kanali where M stands for my name: Manoj Kumar and S stands for my wife name: Sruthi. And my daughter name in her birth certificate is M.S.Kanali. As my daughter is 6 months baby she does not have any other documents as the her proof.

Kindly help me what sould i give as her Given name and Surname in Passport application.
